Midodrine hydrochloride (Midodrine) treats chronic orthostatic hypotension due to autonomic failure in patients with Bradbury-Eggleston syndrome, Shy-Drager syndrome, diabetes mellitus disease, and Parkinson disease.

Medication Pearl of the Day: Midodrine hydrochloride

Indication: Midodrine hydrochloride (Midodrine) may be used to attenuate symptoms of chronic orthostatic hypotension due to autonomic failure in patients with Bradbury-Eggleston syndrome, Shy-Drager syndrome, diabetes mellitus disease, and Parkinson disease.

Insight:

  • Dosing: The usual starting dose of Midodrine is 2.5 mg, 3 times daily.
  • Dosage forms: Midodrine is available as 2.5 mg and 5 mg tablets.
  • Adverse events: Urinary disturbance, pilomotor reactions and supine hypertension.
  • Mechanism of action: Midodrine hydrochloride forms an active metabolite, desglymidodrine, that is an alpha-agonist, and exerts its actions via activation of the alpha-adrenergic receptors of the arteriolar and venous vasculature, producing an increase in vascular tone and elevation of blood pressure.
