This crowded script confused technician Paula J. Thompson, CPhT, of S-Mart Pharmacy in Lodi, California. Although the drug names are not too hard to decipher, the trailing, squished directions present more of a challenge.

Cindy Gochnauer, PharmD, came across this illegible prescription during her shift at Queen Pharmacy in Lancaster, Pennsylvania. The handwriting on this script could have easily led to a medication error without verification from the physician’s office.
