ASCP Announces Forum on Post-Acute Transitions of Care

The American Society for Consultant Pharmacists has announced it will host a discussion for all stakeholders in long-term care.

Alexandria, VA — The American Society for Consultant Pharmacists (ASCP) has announced it will host the “ASCP Forum: Pharmaceutical Care and Post-Acute Transitions,” a discussion for all stakeholders in long-term care on this important issue. This inaugural Forum will provide insights from the nation’s top thought leaders in the post-acute care pharmacy industry. The Forum will be held April 27-28, 2015 at the Baltimore Marriott Waterfront in Baltimore, Maryland. GeriMed and Managed Health Care Associates, Inc. (MHA) are platinum sponsors for the event; Pharmacy Times has signed on as a gold sponsor, and QS/1 is a silver sponsor.

The Forum will bring together high-profile industry experts to provide valuable information on pharmaceutical care options that affect health care providers though all patient care transitions— from the acute to the post-care setting. Presenters will discuss how pharmaceutical expertise can positively affect census as well as decrease readmissions and rehospitalizations following discharge, improving the bottom line. The program is designed for all stakeholders involved in care transitions: consultant pharmacists; health care executives; and managers from long-term care facilities and pharmacy operations, home care, hospitals, and accountable care organizations.

Frank Grosso, RPh, ASCP Executive Director and CEO will moderate the event. “The landscape of our industry is shifting,” said Grosso. “We hear daily how successful care transitions result in improved patient outcomes and minimize additional expense to the whole health care system. Pharmacists must strive to meet the challenge of both improving patient outcomes and minimizing expenses. Both are important and require business relationships among all stakeholders involved.”
