TOOLKITS AIM TO REDUCE DRUG ERRORS
A new set of 17 toolkits to help pharmacists, physicians, nurses, patients, andothers has been developed by the Agency for Healthcare Research and Quality.
The toolkits include checklists to help reconcile medications when patients aredischarged from the hospital, processes to improve effective communicationamong caregivers, and identifying patient risk factors frequently responsible forincorrect medication reconciliation. One toolkit focuses on ways to improve medicationsafety and reconciliation by having pharmacists in hospital emergencydepartments.
