Jaclyn Harth, PharmD: The management of a patient with chronic hyperkalemia is really an interdisciplinary approach. So you want to include physicians, nursing, the patient caregiver, as well as dieticians. And the health-system pharmacist can really work to talk with the patient, counsel the patient on any new medications that they might be started on, making the patient really understands why they’re starting this medication, how to properly take the medication, and how it should be administered.
