That idea became the OTC Guide®, and this year, we mark its 30th edition. What began as a modest survey has grown into one of the most trusted barometers of frontline pharmaceutical expertise in the country. Each year, practicing pharmacists across the nation share their firsthand knowledge of which OTC products they recommend most. The result is a resource built not on advertising budgets or brand recognition, but on real-world clinical experience and patient outcomes.

In 3 decades, much has changed. The OTC landscape has expanded dramatically, with dozens of prescription-strength therapies now available without a prescription. Consumer health literacy has risen. Digital tools have transformed how patients research their symptoms before ever reaching the pharmacy counter. And the role of the pharmacist has evolved alongside all of it—from dispenser to counselor, from back-of-store professional to frontline health care provider.

What hasn’t changed is the pharmacist’s unique position of trust. Study after study confirms that Americans regard their pharmacist as among the most accessible and reliable sources of health care guidance. The OTC Guide exists to honor and amplify that expertise.
