During the discussion, Hall explained what the SHC is and how it impacts health systems, how the SHC’s diabetes DRIVE toolkit can help to address disparities in diabetes disease burden and health outcomes, and what the results of the pilot program assessing the efficacy of the diabetes DRIVE toolkit showed.
