Pharmacy Times® has officially released the 2026 OTC Guide®, marking the 30th anniversary of this industry resource. This year’s guide, compiled from a national survey of 1700 practicing pharmacists, identifies the top recommended brands across 145 product categories, ranging from pediatric care to pain relief. As self-care becomes the primary choice for health management, with over 80% of US adults turning to OTC medicines first for common ailments, the expert guidance provided by pharmacists is more vital than ever.1,2
Nadia Ahmad, PharmD, a participant in the 2026 survey, told Pharmacy Times that patients often enter the pharmacy feeling overwhelmed by the volume of available choices. She explained that pharmacists act as essential guides who help patients "navigate all through these complexities and questions they have, and sometimes [turn] confusion into clarity." For Ahmad, the guide is significant because it represents more than just a list of products; it reflects the "knowledge, experience, and trusted care we provide to our patients every day."
The 2026 results highlight several standout brands that have earned the "#1 Pharmacist-Recommended" designation. In the Gastrointestinal category, Imodium secured an impressive 81% recommendation rate for antidiarrheals, while Pepcid led H2 receptor antagonists with 83%. The Cough, Cold, and Allergy section featured dominant performances from Flonase Allergy Relief, which earned top marks for both intranasal allergy products (77%) and intranasal corticosteroids (82%). For nutritional support, Nature Made remained a powerhouse, leading in several categories including "Letter" vitamins (66%) and Coenzyme Q10 supplements (51%).1
Beyond identifying market leaders, the guide serves as a critical benchmark for health care professionals. Ahmad noted that in a "busy practice setting," especially in community pharmacies that can become "very hectic," the guide is a necessary tool. By seeing what their peers across the country recommend, pharmacists can reinforce their own clinical decisions or discover new products that might better serve their local demographic.
"This OTC guide also helps us all as pharmacists to make informed [decisions], and it supports evidence-based recommendation and ultimately helps provide the best possible care to our patients," she said.
The economic and clinical value of these recommendations is profound. Research suggests that for every $1 spent on an OTC product, more than $7 are saved in cost avoidance for the health care system.1 By recommending trusted brands like CeraVe for therapeutic skin care (32%) or Excedrin Migraine (65%), pharmacists provide accessible relief for an estimated 60 million individuals who might not otherwise seek medical care.
As the 30th edition of the OTC Guide demonstrates, the unique position of trust held by pharmacists remains unchanged. As Ahmad concluded, having this "reliable tested source available once a year" ensures that the frontline expertise of pharmacists is both recognized and utilized: "Our voices are heard and we can make a recommendation that can be printed and published and available for our review."
