Expert Reviews Common Medication Errors Involving Direct Oral Anticoagulants
Pharmacists can play an important role in avoiding common medication errors for patients receiving direct oral anticoagulants.

In an interview with Pharmacy Times, Zachary Noel, PharmD, BCCP, discussed the presentation at the American Society of Health-System Pharmacists 2021 Midyear Meeting titled "Direct Oral Anticoagulant Dilemmas: Stroke Prevention for Atrial Fibrillation in the Oldest Old and Patients with End-Stage Renal Disease."
In the interview, Noel discussed the most commonly seen medication errors for patients receiving direct oral anticoagulants, including patients ages 80 and older and those with end-stage renal disease.
