The recent expansion of HPV self-collection options, including an FDA-cleared at-home kit and updated federal guidelines, is the most significant shift in cervical cancer screening in years. Pharmacists are positioned to play a meaningful role in patient education and access.

What Has Changed in Cervical Cancer Screening Options?

In May 2025, the FDA approved the first at-home self-collection cervical cancer screening kit—significantly expanding access to testing that otherwise may not be accessible for some populations. Federal guidelines released in January 2026 by Health Resources and Services Administration state that individuals with a cervix aged 30 to 65 with average risk of cervical cancer can opt for a self-administered HPV test, bringing self-collection into the standard-of-care framework for the first time.1

How Accurate Is Self-Collected HPV Testing Compared With Clinician-Collected Samples?

Research shows comparable performance for detection of high-risk HPV when FDA-authorized self-collection methods and validated assay-device combinations are used. Guidelines developed through a collaboration of 19 national organizations advise clinicians and patients on how to manage results from self-collected testing, a recognition that clinical workflows must adapt alongside the testing options themselves.2

This includes patients who face geographic barriers to clinic access, patients who find speculum-based exams uncomfortable or distressing, and those with limited availability to schedule and attend appointments. Pharmacists who dispense or counsel around women's preventive care can identify candidates and facilitate access through telehealth referral or prescription pathways.1

Does a Positive Self-Collected HPV Test Mean a Patient Has Cancer?

In short, no—and this is a critical counseling point. A positive HPV result indicates the presence of high-risk HPV types, not a cancer diagnosis. A positive HPV result means additional testing is necessary, which is typically a colposcopy or follow-up per ASCCP management guidelines. Pharmacists and other healthcare providers who discuss results with patients should frame positivity as a call to action, not a diagnosis, and ensure patients understand the follow-up pathway before testing.1

What Should Pharmacists Know About the Different Approved Testing Platforms?

Not all HPV tests and self-collection devices are interchangeable. The ACS cervical cancer screening guideline includes only FDA-approved combinations of HPV assays and collection devices, and only specific pairings have been validated for self-collection. One currently approved at-home option is the Teal Wand, used with the Roche cobas HPV assay and accessed through a proprietary telehealth network.3

Additional platform combinations are under evaluation in ongoing SHIP trial substudies. Pharmacists should not assume that an HPV assay approved for clinician collection is validated for self-collected specimens without verifying the specific device-assay pairing.3

What Is the Pharmacist's Role in Advancing Cervical Cancer Screening and Prevention?

Self-testing for cervical cancer screening allows people to test themselves privately in the comfort of their own homes, which can directly reduce the barrier of clinic avoidance. Pharmacists in retail, specialty, and ambulatory settings are often the most accessible health care professionals in a patient's life—and are well-positioned to identify patients overdue for screening, explain the difference between self-collection options, and connect patients to appropriate prescribing or telehealth resources.4

Vaccination is the most effective way to protect against cervical cancer and HPV viruses. The HPV vaccine, particularly Gardasil, has driven steep declines in the HPV types it targets—infections fell 88% in females aged 14 to 19 and 81% in those aged 20 to 24. Pharmacies can administer the HPV vaccine to individuals who meet the recommended age requirements. In some states, a prescription from a doctor may be warranted.5-7

In settings where pharmacists can order or recommend preventive services such as self-testing or vaccination, screening and immunization rates have been shown to improve—underscoring the value of expanding pharmacists' scope of practice in cervical cancer prevention.
