Centennial Pharmacy Services in Pennsylvania Highlights a Lifelong Passion for Patient Care

Lindsay Dymowski Constantino is a co-owner and president of Centennial Pharmacy Services in Philadelphia, Pennsylvania, along with her brother Joe Dymowski and parents Cindi Dymowski and Joseph Dymowski, PharmD. Initially, Constantino had planned to follow in her father’s footsteps and attend pharmacy school at Temple University. However, her experiences led her in a different direction.

“Growing up with my parents’ community pharmacies, I was deeply involved in the daily operations, which gave me a strong foundation in patient care and medication management,” she said. “That experience instilled in me a lifelong passion for patient care and innovative pharmacy services.”

Throughout college, Constantino worked in her family’s pharmacies, seeing firsthand the need for more effective medication management strategies. Inspired by articles on the evolving role of pharmacists in health care—especially in areas like improving outcomes through appointment-based models and medication synchronization—she pivoted. Rather than continuing with formal pharmacy education, she invested her tuition money in purchasing an adherence packaging robot to enhance the pharmacy’s focus on comprehensive medication management.

Constantino’s approach to pharmacy is unique. The pharmacy team works closely with health care providers, offers comprehensive medication reviews with every prescription, and educates patients. They focus on building trust within the local community and providing each patient with the care they need right at home.

“Our pharmacy isn’t just about dispensing medications. It’s a care hub where every prescription is an opportunity to enhance a patient’s health journey,” Constantino said.

Technology plays a vital role in Centennial’s operations. Constantino credits several tools that enhance their services, including Parata PASS packaging technology to improve adherence; Parata Perl for accuracy, visual verification, and medication history tracking; a proprietary workflow management technology that tracks patient statuses in real time; and Rx4Route, which optimizes delivery routes and schedules.

Meanwhile, key services Centennial offers include chronic care coordination to help patients manage chronic conditions in collaboration with other health care providers; personalized delivery and education; 24/7 pharmacy access; and additional provider partner services such as vaccinations, diabetic education, tobacco cessation programs, blood pressure reviews, and point-of-care testing.

“Centennial is well known and highly recommended for our specialized medication management services,” Constantino said. “Many patients are referred by nurse managers, care coordinators, or health care providers.”

Like many independent pharmacies, Centennial is known for its personal touch. Each new patient receives a welcome package with information about available services. The pharmacy team ensures patients are seen by the same care team each time, which Constantino says “not only makes our patients feel valued but also enhances the quality of care we provide.”

What sets Centennial apart, Constantino believes, is their patient-centered, long-term care-at-home medication management model. The pharmacy specializes in providing comprehensive care to patients who require support typically found in long-term care facilities but who prefer to remain at home “Our dedication to at-home care allows us to meet our patients’ unique needs by providing not just medications but a full support system,” she said.

Constantino recalls a memorable patient who was taking more than 30 medications when he first came to Centennial. Overwhelmed and nonadherent, he had multiple hospital readmissions. The pharmacy team started him on a medication management program, using prepackaged medications organized by time and date, along with regular check-ins. His health stabilized, allowing him to cut his medications in half, avoid hospital visits, and enjoy an improved quality of life. This is just one example of how Centennial Pharmacy Services supports patients.

“At the heart of our pharmacy is a commitment to being community focused and patient centered,” Constantino said. “This mission is the cornerstone of everything we do at Centennial Pharmacy Services.”

To connect with the local community, the pharmacy team offers lunch-and-learn sessions for health care professionals, fostering collaboration between pharmacists and providers. These sessions, she explained, help bridge the gap between pharmacy services and broader health care practices, creating a more collaborative environment that ultimately benefits the patient. Constantino notes that these sessions often have an immediate impact, with health care providers gaining a clearer understanding of how pharmacists contribute to the care team.

In her spare time, Constantino enjoys spending time with her family—her husband and 2 sons—and looking forward to the arrival of their third child. She also enjoys spending time with her parents and brother. Now living in Naples, Florida, Constantino enjoys the sunshine and loves being outside every day, whether at the beach or zoo with her family. She also enjoys gardening, though she jokes that she lacks a green thumb.

“What I lack in skill, I make up for in persistence,” she said. “There’s something incredibly rewarding about seeing tangible results sprout from the soil. Even better, my kids actually eat vegetables if they’ve yanked them out of the ground themselves!”
