Grant Will Help California Senior Program
Los Angeles seniors will benefit from a 3-year grant that will bring a successful model for managing seniors' medications. With $600,000 in funding from the US Department of Health and Human Services Administration on Aging, Partners in Care Foundation's Medications Management Program will increase access to medications screening and review as an early preventive measure to help avoid medication errors.
Partners in Care Foundation started and tested a successful Medications Management Model through home health agencies nationwide. The new grant will adapt the evidence-based model to local aging services providers.
The goal of the intervention is to identify, prevent, and resolve potential and real medication-related problems among seniors identified as at high risk.
