Enrollment Now Open for ACCESS to Pharmacy Care Program

ALEXANDRIA, Va. (Sept. 14, 2026) – Enrollment is now open for ACCESS to Pharmacy Care, a National Community Pharmacy Association (NCPA) program administered by CPESN® in partnership with GamePlan Medical, a CMS ACCESS participant. The program unlocks the full potential of community and long-term care pharmacy in outcomes-based care service delivery by supporting participation in the CMS ACCESS model.

The program is open to NCPA members and CPESN pharmacies and is designed to fit into existing pharmacy workflows while creating new revenue opportunities tied to chronic condition care and measurable patient outcomes.

“ACCESS to Pharmacy Care represents a defining opportunity for pharmacy to demonstrate its value in improving patient outcomes at scale,” said NCPA CEO B. Douglas Hoey, pharmacist, MBA. “This program creates a practical pathway for pharmacies to deliver the kind of ongoing chronic condition support patients need, while building a sustainable model for pharmacy-delivered care.”

Through the CMS program, participating pharmacies help deliver ongoing, technology-supported care for eligible Original Medicare patients with chronic conditions including high blood pressure, diabetes, high cholesterol, kidney disease, heart disease, chronic pain, anxiety, or depression. The CMS ACCESS Model is designed to shift payment toward patient outcomes, expand access to chronic care support, and enable continuous, team-based care beyond traditional office visits. Pharmacies receive payment for services through the GamePlan Medical collaboration.

Several pilot pharmacies partnered with GamePlan Medical over the past few weeks to refine workflows that support the delivery of additional active care services to patients, as outlined in the ACCESS Model. These efforts included streamlining documentation processes, enhancing patient engagement strategies, and strengthening care coordination support to help ensure success as enrollment opens to NCPA members and CPESN pharmacies nationwide.

GamePlan Medical serves as the CMS ACCESS participant and clinical coordinating entity, providing medical director oversight and partnering with community pharmacies to extend care delivery into patients’ communities. CPESN and NCPA support pharmacy participation through training, tools, onboarding, and ongoing guidance designed to help pharmacies succeed in the outcomes-based model.

Initial participants also helped validate a readiness-based approach for pharmacy enrollment. Some pharmacies may be ready to begin immediately, while others may use NCPA and CPESN training and support to strengthen medication synchronization, appointment-based care, documentation, and clinical services delivery before fully engaging in the CMS innovation model.

“This is not a program pharmacies have to figure out alone,” said CPESN Executive Director Troy Trygstad, PharmD, MBA. “NCPA, CPESN, and GamePlan Medical have worked together to centralize the administrative, technical, and clinical coordination so pharmacies can focus on patient care.”

ACCESS to Pharmacy Care comes at a time when more than two-thirds of Medicare beneficiaries live with at least one chronic condition, and care often remains fragmented between provider visits. The program builds on pharmacy’s frequent patient touchpoints and trusted local relationships to support better outcomes for patients living with chronic disease.
