Clinical Support Services, Inc. to Provide Medication Management Software to Rite Aid's Health Alliance Program

BUFFALO, N.Y., Aug. 12, 2014 /PRNewswire/ -- Clinical Support Services, Inc. announced today that they have entered into an agreement with Rite Aid, one of the nation's leading drugstore chains. Through the agreement, CSS will provide MTMPath medication management software to Rite Aid pharmacies piloting Rite Aid's new Health Alliance program.

Unveiled in March of this year, Rite Aid's Health Alliance program offers coordinated, comprehensive care and support to patients with chronic and poly-chronic conditions like congestive heart failure, diabetes, COPD, hypertension and high cholesterol. Partnering physicians and Rite Aid pharmacists work with specially trained care coaches, located in select Rite Aid pharmacies, to achieve specific physician-identified wellness goals and improve the patient's overall health and self-management abilities.

Comprehensive medication reviews by Rite Aid pharmacists is one of the key services available to patients participating in Rite Aid Health Alliance. Rite Aid pharmacists will use the MTMPath software to document the medication review; this allows the information to be collected in a standardized way and stored in an easy-to-access database for all members of the patient's care team.

Additionally, MTMPath generates a variety of reports that the pharmacist can use to guide the patient consultation and provide documentation to share with the physician and use for follow-up visits or conversations. The documentation enables Rite Aid pharmacists to easily communicate the details of the medication review with the patient's physician as well as share pharmacist recommendations for therapeutic improvement.

By documenting patient medication information, and sharing data between all members of the care team through MTMPath software, patients can experience improved health outcomes, as they will have a better understanding of their conditions and how to manage them.
