Rx 3: The Rx is for 2 sisters, H— — and S— —. Elixir of decadron 90 cc: H— — is to take 0.75 tsp 3 times a day for 6 days and S— — is to take 1 tsp 3 times a day for 6 days

Rx 4: Flexeril 10 mg #30, sig 1 PO bedtime PRN
