Sam Brian, Jr, RPh, could not make heads or tails of this messy script, which arrived in a stack of equally illegible prescriptions at Court House Pharmacy in Winnfield, Louisiana. A call to the physician’s office cleared things up for Sam.

The writing on this prescription slip looked more like hieroglyphics than handwriting to Nick Simone, RPh, of Bonus Drug & Surgical in Astoria, New York. He was unable to fill the prescription until he spoke with the physician directly.
Can you read it?
ANSWERS
Rx 1: Flexeril Tab 5 mg--#100, take 1 every 6 hours as needed for muscle spasms, no refill. Neurontin 400 mg--#100, take 1 by mouth 3 times daily for pain, no refill.
Rx 2: Promethazine with codeine--#120mL, take 1 teaspoonful 4 times daily.
