This prescriber is considered a repeat offender by pharmacists and technicians at the Walgreens pharmacy in Okolona, Kentucky. Despite their considerable experience deciphering the physician’s handwriting, pharmacist Traci Walls, RPh, and her colleagues simply could not decode this script. Can you read it?

When a patient arrived with this prescription at Kerr Drug in Benson, North Carolina, Melissa Johnson, PharmD, and Brittany Moody, CPhT, decided to call the physician’s office for clarification. The answer they received proves that even the simplest prescriptions can be muddled by subpar penmanship. Do you know what it says?

Answer Rx 1: 1) Humulin 70/30—U-100, 5 vials, 60 units every morning. 2) Tricor 145 mg—#90, take 1 by mouth at bedtime, refill 1 time.

Answer Rx 2: Prenatal vitamins #90—take 1 tablet daily.
