INNOVATIVE MTM PROGRAM WILL HELP SENIORS
Outcomes Pharmaceutical HealthCare has been chosen by TrilliumCommunity Health Plan to administerthe medication therapy management(MTM) services component of the 2007Trillium Advantage Medicare Part Dprescription drug plan in Oregon.
As part of the plan, enrollees willreceive MTM services from speciallytrained Personal Pharmacists to helpthem reach safe and effective resultsfrom their medications while controllingcosts. New services that areavailable to members include theannual Medication CheckUp, whichallows members to meet individuallywith a pharmacist in their area. Thepharmacist will evaluate the patient'scomplete medication profile to spotany potential conflicts or duplications.Additional services availableinclude patient education, adherencemonitoring, and help with OTC medications.
