Guidelines for Drug Product Naming

Michael Cohen, RPh, MS, ScD, DPS, FASHP, President of the Institute for Safe Medicine Practices (ISMP), discusses the guidelines in place to avoid having drug products with similar names. He also discusses instances in which product names have been the source for error.
