Hilda Galvan, certified pharmacy technician, spent several minutes pondering this 5-part prescription with colleague and pharmacist Sreedhar Vajuinepalli of Health Care Pharmacy in Passaic, New Jersey. Unable to decipher it, they took action and called the physician’s office for more information.

Catherine Dolcimascolo, PharmD, was presented with this prescription at Osco Pharmacy in Arlington Heights, Illinois. Fortunately, Catherine did not have to track down the prescriber to clarify the prescription. She figured it out on the spot by speaking with the patient instead.
