John Rothwell, PharmD, manager of the Walgreens pharmacy in San Antonio, Texas, struggled to read this prescription. With so many important details crammed into the margins and squeezed between lines, Rothwell decided a call to the physician’s office was in order.

This prescription stumped Mayer Maltz, RPh, pharmacist at the New Utrecht Pharmacy in Brooklyn, New York. Unable to decipher it on his own, he spoke with the physician to confirm the prescription before filling it.
