TOOLKITS AIM TO REDUCE DRUG ERRORS

A new set of 17 toolkits to help pharmacists, physicians, nurses, patients, and others has been developed by the Agency for Healthcare Research and Quality.

The toolkits include checklists to help reconcile medications when patients are discharged from the hospital, processes to improve effective communication among caregivers, and identifying patient risk factors frequently responsible for incorrect medication reconciliation. One toolkit focuses on ways to improve medication safety and reconciliation by having pharmacists in hospital emergency departments.
