can you READ these Rxs?

Pharmacist Charles DeBoyce and staff, of Price Chopper Pharmacy #134 in Oneonta, New York, were not sure what medication was being prescribed when the patient dropped off this script at the counter. After a call to the prescribing physician's office for clarification, DeBoyce learned that the drug being ordered is not commercially available in the strength prescribed. Can you figure out what this medicine is?



Registered Pharmacist Cecil Shern, of Cecil's Pharmacy in Lehi, Utah, was stumped when this prescription was brought into the pharmacy. The patient was unable to provide any clues for this mystery script. Shern called the prescribing physician's office for verification. The physician's nurse was able to unravel the mystery. Can you decode this prescription?
